Finance

No worthwhile Signal in this section. The CMS and TMHP newsrooms had no new payment-rate item for physician groups since the October 15 edition on the 2027 conversion factor, and the final 2027 fee schedule rule had not appeared in the searches run.

RCM

No worthwhile Signal in this section. The one new Texas Medicaid billing item, a peer specialist code change, is covered under Worth Your Time because it is narrow.

Operations — Lead: Medicare Advantage directory accuracy becomes a plan scorecard item

On October 16, CMS proposed reforms to Medicare Advantage and Part D (CMS press release, October 16, 2026). Under the proposal, Medicare Advantage plans would have to verify and update provider directory information at least every 90 days, report an accuracy score to CMS, and display that score on their public directories. Enrollees who get care from an out-of-network provider because of an inaccurate listing would pay no more than in-network cost sharing. The proposed changes apply to contract year 2028.

The CMS fact sheet on CMS-4214-P (October 16, 2026) says the provider directory provisions implement section 6220 of the Consolidated Appropriations Act, 2026. It adds that in contract year 2029 CMS must publicly display accuracy scores in a machine-readable format. Neither CMS page describes specific obligations for physicians or practices.

Why it matters: this is a proposal, not a final rule, and it binds plans, not practices. But plans that must verify every 90 days will likely contact practices more often to confirm locations, specialties and who is accepting patients. Practices should expect that outreach and decide now who owns the answer.

Who and when: Medicare Advantage plans and the practices listed in their directories, nationally. Proposed for contract year 2028. CMS says the comment period runs 60 days after Federal Register publication; the exact deadline was not given on the CMS pages.

Worth Your Time

  • RCM — Benefit Updates for Texas Medicaid Peer Specialist Services Effective December 1, 2026 — TMHP, October 16, 2026. Managed care plans may have their own prior authorization and claims rules. Relevant mainly to groups with behavioral health programs.

What to Watch

  • Federal Register publication of CMS-4214-P, which starts the 60-day comment clock.
  • The final 2027 Medicare Physician Fee Schedule rule, which has typically come in November.
  • MGMA Stat polls on Tuesdays and Thursdays for practice-side reaction.
