Finance
No worthwhile Signal in this section. Today's check of the CMS newsroom (listing, October 19, 2026), MGMA Stat (listing, latest posts October 6 and 13, 2026) and the FTC press release page (listing, October 19, 2026) found nothing new on reimbursement, deals or labor beyond what recent editions covered.
Why it matters: The October 15 edition's 2027 conversion-factor story is still the live Finance item, and the final rule is pending.
Who and when: Not applicable today.
RCM — Lead: UnitedHealthcare drops prior authorization on about 30% of its requirements
UnitedHealthcare's provider notice, Prior authorization reductions (September 1, 2026), says it is eliminating 30% of prior authorization requirements starting October 1, 2026. It covers commercial, Medicare Advantage (including D-SNP), Community Plans, Individual Exchange and Oxford plans. UnitedHealthcare publishes a separate PDF for each plan type and points providers to its Prior Authorization and Notification Tool to check a specific patient or service.
The notice itself does not give a code count or service categories. Trade press does. Healthcare Dive (September 2026) reports about 1,700 CPT codes, including cardiology, oncology, orthopedics, genetic and lab testing, therapy, home health and durable medical equipment. It reports the count by plan (more than 800 commercial, about 940 ACA, about 120 Medicare Advantage and dual special needs) and says Medicaid varies by state, from 3 codes in Washington, D.C. to more than 600 in Texas. Those counts are Healthcare Dive's reporting; the Texas figure and which Texas plans it covers are not confirmed on UnitedHealthcare's page, so check the Community Plans PDF.
Two caveats. The 30% is a share of UnitedHealthcare's prior authorization requirements, not of services. And Medicare Advantage is a small slice of the change, per Healthcare Dive's plan breakdown.
Why it matters: Staff who routinely request authorization for the dropped codes may be doing work that is no longer required, and front-desk scripts, scheduling holds and denial-prevention checklists built around old rules can be trimmed. Dropped requirements also don't cover every plan or patient, so the lookup tool, not memory, should decide.
Who and when: UnitedHealthcare commercial, Medicare Advantage, Community, Individual Exchange and Oxford plans; effective October 1, 2026. Texas relevance rests on the Healthcare Dive count and should be confirmed in the Community Plans PDF.
Operations — Texas Medicaid peer specialist services expand to age 14 and telehealth
TMHP's news listing (October 16, 2026) carries a notice, "Benefit Updates for Texas Medicaid Peer Specialist Services Effective December 1, 2026." Per that notice, peer specialist services (procedure code H0038) expand from clients 21 and older to clients 14 and older, and the manual's in-person-only statement will be removed so the service can be delivered by telehealth. Billing uses H0038 with modifier 95 for synchronous audiovisual or modifier FQ for audio-only. The notice does not mention rates. Managed care members should check their MCO for authorization and claims rules.
Why it matters: Behavioral health and integrated primary care groups that bill H0038 may need template, scheduling and modifier updates before December 1. The notice does not say how many practices bill this code.
Who and when: Texas Medicaid providers billing H0038; dates of service on or after December 1, 2026.
Worth Your Time
- RCM — FTC Issues Letters Warning Hospitals Against Deceptive Pricing Practices — FTC, October 5, 2026. The FTC sent letters to 24 large healthcare services companies saying CMS price transparency rules do not create a safe harbor from FTC Act liability. It is not a finding against any recipient. Mostly a hospital-system story, but a reminder that price estimates and disclosures draw attention beyond CMS.
- RCM — The unhappy surprise of No Surprises — HFMA, October 19, 2026. A trade-association post; the host interviews a columnist who argues the No Surprises Act is driving medical inflation. That is a stakeholder view, and the page gives no data.
What to Watch
- The final 2027 Medicare physician fee schedule, which Medical Economics (October 1, 2026) says CMS will release in November.
- Whether other Texas payers follow UnitedHealthcare's prior authorization cuts, and whether practices report fewer PA denials after October 1.
